Duodenal switch packs a one-two punch against obesity. It does so by combining two surgical techniques: restrictive and malabsorptive..

The restrictive component involves reducing the size of the stomach. Your bariatric surgeon would divide the stomach vertically and remove more than 85 percent of it. The stomach that remains is shaped like a banana and is about 100 to 150 milliliters or 6 ounces.

Duodenal switch surgery is a variation of another procedure, called biliopancreatic diversion. But the duodenal switch leaves a larger portion of the stomach intact, including the pyloric valve, which regulates the release of stomach contents into the small intestine

As the name suggests, the duodenal switch also keeps a small part of the duodenum in the digestive system. The duodenum is the first part of the small intestine. It is located between the stomach and the jejunum, or the middle part of the small intestine. Foods mix with stomach acid, then move down into the duodenum, where they mix with bile from the gall bladder and digestive juices from the pancreas.

Malabsorptive surgeries restrict the amount of calories and nutrients the body absorbs. The malabsorptive component of duodenal switch surgery involves rearranging the small intestine to separate the flow of food from the flow of bile and pancreatic juices. The food and digestive juices interact only in the last 18 to 24 inches of the intestine, allowing for malabsorption. Unlike the restrictive part of the surgery, the intestinal bypass part of the duodenal switch is partially reversible if you are one of the people who experience malabsorptive complications.

With the duodenal switch, you consume less food than normally, but it is still more than with other weight loss surgeries. Even this amount of food cannot be digested as normal, so a large amount of food passes through the shortened intestines undigested.

The duodenal switch can also be performed laparoscopically, meaning that your surgeon makes small incisions as opposed to one large incision. He or she inserts a viewing tube with a small camera (laparoscope) and other tiny insert instruments into these small incisions to perform the duodenal switch procedure.

Bariatric Surgery Treatment - Gastric Sleeve

Put another way, gastric sleeve surgery lets such people reach a safe weight so they can then undergo the more radical gastric bypass surgery or duodenal switch surgery. The American Society for Metabolic and Bariatric Surgery states that this is the most useful application of gastric sleeve surgery at this time, because it appears to be faster and/or easier than other weight loss surgeries in these high-risk people.

Other names for gastric sleeve resection are sleeve gastrectomy, vertical sleeve gastrectromy, tube gastrectomy and laparoscopic sleeve gastrectomy.

During this procedure a bariatric surgeon removes about 60 percent of the stomach so that it takes the shape of a tube or sleeve

This operation is performed laparoscopically, meaning that the surgeon makes small incisions as opposed to one large incision. He or she inserts a viewing tube with a small camera (laparoscope) and other tiny instruments into these small incisions to remove part of the stomach. The tube-shaped stomach that is left is sealed closed with staples.

It is often followed by a gastric bypass or duodenal switch after the patient has lost a significant amount of weight. Called a "staged" approach to weight loss surgery, this makes the second procedure less risky than it would have been had it been the first and only procedure.

The timing of the second surgery varies according to the degree of weight loss. It usually occurs within six to 18 months.

Bariatric Surgery Treatment - Band Gastric Surgery

Adjustable gastric banding is a restrictive type of weight loss surgery. In a nutshell, it involves placing a silicone band with an inflatable inner collar around the upper stomach to restrict food intake. This creates a small pouch and a narrow passage to the lower stomach. This small passage delays the emptying of food from the pouch and causes a feeling of fullness. The silicone band can be tightened or loosened over time to change the size of the passage.

Other terms for adjustable gastric banding are laparoscopic adjustable gastric banding and gastric banding. Brand names of gastric banding devices include the Realize Band and the Lap-Band System.

The silicone band is connected to a small port that is placed in the abdominal wall. The inner diameter of the band can be adjusted by injecting saline through this port. This is called a fill. Your surgeon can make periodic adjustments or fills based on your weight loss, food cravings and other physical reactions to the surgery.

Adjustable gastric banding is usually performed laparoscopically, meaning that the surgeon makes small incisions as opposed to one large incision. To perform the gastric banding, he or she inserts a viewing tube with a small camera (laparoscope) and other tiny instruments into these small incisions.

Currently two types of gastric bands are on the market in the United States: the Lap-Band System and the newer Realize Band.

Gastric banding surgery takes about one to one and a half hours. It can be done on an outpatient basis or may require a one-day hospital stay.

Bariatric Surgery Treatment - Band Lap Surgery

Lap-Band surgery is a type of weight loss surgery that, like gastric bypass, alters the anatomy of the stomach to help you reduce food intake.

"Lap-Band System" is the brand name of the FDA-approved, adjustable gastric band used in the procedure. Most simply, a Lap-Band is a silicone belt that goes around the top of the stomach. This results in the ability to control your hunger and achieve a "feeling of fullness," or satiety. Ultimately, you eat much less and should feel full sooner.

A key component of the Lap-Band System is its adjustability. The band is connected to a port, which enables the device to be inflated over time to expedite the weight loss.

After performing a series of small incisions, your surgeon would use a small camera, called a laparoscope, to visualize placement of the Lap-Band. The Lap-Band is placed around the top of the stomach and secured in place with sutures. The port is then placed underneath the skin on the top part of the abdomen. Surgery should take only about an hour, and an overnight stay in the hospital may or may not be required.

Injecting saline into the port inflates the band, making the passage to the lower part of the stomach smaller, which further restricts food intake. Fluid can easily be removed in the doctor's office if it is too tight.

Following surgery, you should be able to return to work within a week, with minimal discomfort. You will need to follow a nutrition plan (which may include vitamin supplements) prescribed by your surgeon and/or nutritionist. The nutrition plan will likely include a liquid diet for a few weeks, until you can tolerate soft foods, and later solid foods. A specific exercise program, as well as behavioral-modification therapy, may also be recommended.

Frequent Hospital visits are mandatory to adjust the band. Your surgeon or his assistant will be able to inject the port in the office. On average, patients require six to eight adjustments of the band during the first 18 months following Lap-Band surgery.

Patients typically lose 50% to 60% of their excess body weight within two years after the procedure. Additional plastic surgery may be indicated, depending on how much weight is lost.

It is a complex procedure performed under general anesthesia — you sleep through it. Time in surgery is approximately one to four hours, followed by a one- to seven-day stay in the hospital. Currently, two techniques are available for gastric bypass:

    Roux-en-Y-gastric bypass (traditional and laparoscopic)
    Biliopancreatic diversion bypass

Roux-en-Y-Gastric Bypass (Traditional and Laparoscopic)

The Roux-en-Y bypass is more common and considered less complicated than the biliopancreatic diversion bypass, since Roux-en-Y does not remove portions of the stomach.

The traditional Roux-en-Y-gastric bypass is performed through open surgery with one long incision.

The Roux-en-Y gastric bypass can also be performed laparoscopically. The laparoscopic Roux-en-Y-gastric bypass uses multiple smaller incisions (instead of one long incision) a laparoscopic tool is inserted, which offers a visual guide to the inside of the abdomen during the procedure.

The laparoscopic Roux-en-Y produces less scarring and lets you recover faster than the traditional Roux-en-Y-gastric bypass. But the laparoscopic approach is still new, so long-term results have not been fully evaluated.

Biliopancreatic Diversion Bypass
The biliopancreatic diversion bypass is performed through open surgery with one long incision, leaving a permanent scar. It is less common and more complicated than the Roux-en-Y-gastric bypass.

In the biliopancreatic diversion, portions of the stomach are removed and the bypass is attached to the distal illium. This procedure is not widely used, because there is more risk of nutritional deficiencies.
